When a Premera member with Type 2 diabetes, arthritis, and other chronic conditions started missing important follow-up care, our systems identified a pattern that needed attention. There was no clear crisis, but there was an opportunity to step in early and help.
A Premera case manager reached out, listened to the member’s concerns, and helped identify practical next steps. The member had stopped taking a prescribed medication because of side effects and was struggling to keep up with appointments around work. The case manager helped the member reconnect with care, enroll in digital diabetes support, set realistic health goals, and prepare questions for their provider.
That support became even more important when an emergency room visit led to a thyroid cancer diagnosis. The case manager continued to stay close, helping the member navigate appointments, resources, pre-surgery planning, recovery needs, and ongoing pain management. Just as important, the case manager offered steady, human support during a frightening and complex health journey.
With help from their case manager, the member stabilized their blood sugar, stayed connected to their care team, completed surgery, and continued making progress toward better health. It’s a powerful example of how proactive, coordinated support can help members move from uncertainty to action.
