How to Make Changes to Chronic Condition Management

Last year Chronic Condition Management Plus (powered by Teladoc) launched for groups looking to support their members living with chronic conditions like diabetes and hypertension. For the 2026 plan year, a renewal form is required for self-funded groups if:

• The group is new to Chronic Condition Management Plus
• The group is changing any program selections
• The group is terminating any existing Chronic Condition Management Plus programs
